When you intend to travel with your pet, there are a number of things you need to take into consideration. One of these is the health of your pet. Before you can bring your pet on any international travels, you need to receive a pet health certificate form from your veterinarian. Different countries all over the world have different forms. In the United States, the form is put out by the Department of Agriculture.

What Is a Pet Health Certificate Form PDF Used For?

In the United States, if you intend to travel between states with your pet, or you intend to bring your pet on international travels, you need to receive a health certification from your veterinarian. This health certification shows that your pet is safe to travel. To receive a certification, two things must be true:
    • Your pet's current physical health is good enough to handle the potential stresses of travel

  • Your pet does not have any potentially communicable diseases and is up-to-date on all vaccinations and health treatments

Who Would Use a Pet Health Certificate Form PDF?

Pet health certificates are used by pet owners who intend to bring their pets on travels with them. The certificate is a way of verifying that a pet is safe to travel. Not every country has an official pet health certificate, but many do. Each is unique to the country of origin, but all contain the same basic information: It is safe for the pet to engage in international travel. A pet's owner is the one who needs the certificate. However, the owner is not the one who fills out the certificate. Instead, the veterinarian must issue the certificate. Because the certificate is only valid for 30 days, the veterinarian should complete and issue it as close to the proposed travel date as possible. If a person is bringing multiple pets, one certification can record up to six individuals. However, for a veterinarian to sign off, they must have examined all pets listed on the certificate.

When Should You Use a Pet Health Certificate Form?

If you intend to engage in international travel with your pet, a pet health certificate is imperative. Some countries, particularly those in the European Union, have their own pet health certificates. When this is the case, you should use these forms. However, if your destination does not have a certificate, United States travelers should use the standard international United States certificate. If you're traveling within the United States, but you are engaging in interstate travel by plane, you should also bring a pet health certificate. You can show this to any relevant security people to prove that your pet is safe to come along. Having your pet examined before potentially rigorous travel is just a part of responsible pet ownership. For veterinarians, it's easy to update the certificate information and email it to clients. You can print the PDF of the certificate and bring it with you for your veterinarian to fill out. Alternatively, you can bring your tablet or phone so your veterinarian can fill it out electronically. It's very important that you explain when and where you intend to travel. Your veterinarian can make travel recommendations for your pet based on their age and health.

What Are the Consequences of Not Using a Pet Health Certificate Form?

If you don't have a valid pet health certificate, you might not be permitted to travel with your pet. If your veterinarian won't sign the certificate due to your pet's health issues, you'll need to find a petsitter to take care of your pet while you're away.

How to Fill Out a Pet Health Certificate Form

    • Check the box next to the type of animal being shipped. Fill out the total number of animals and number of certificate pages you need. The owner can write their name, address, and contact information. The name, address, and contact information of the recipient must also be filled out.

    • Either the owner or the veterinarian can fill out identifying information about the pets being shipped. Up to six can fit on one form. You'll need to include name, breed, age, sex, distinctive marks, and whether or not they have a microchip.

    • The veterinarian should fill out any additional animal comments, if applicable. Then, they should fill out the section for pertinent vaccination, testing, and treatment history. This includes rabies vaccinations, other vaccinations, treatments, and test results.

    • The veterinarian should check the appropriate boxes for certification.

  • The veterinarian should fill out their name, address, and contact information. They should also include their license number and state, national accreditation number, and signature.

Quick Questions

You can print and bring the PDF to your veterinarian office for free. From there, the cost will depend on what potential treatments your pet needs. You'll need to pay for the examination, any vaccinations, and any other medical expenses.

Yes. According to United States law, every pet that crosses a state border must have a health certificate and rabies immunization. The certificate must have been issued by a veterinarian 30 days or less before travel.

The laws differ from state to state. Most states require pets to have a health certificate before they can cross the border. There are some states that don't require a health certificate, but do require proof of a rabies vaccination.
