How to Fill Out an SSA 3373 PDF Form
Section A will record general information about you. You'll need to note your full name, SSN, and the daytime telephone number at which you can be reached. If you don't have a phone number at which you can be reached, you'll need to provide a number at which a message can be left for you. You'll need to put a check mark by any listed items that your disability affects. For any checked item, you'll need to provide an explanation.
